Trialists have tended to think that the end of the trial is the time to think about implementation, if they think about it at all.  I think this is a mistake.  I’ll argue that the time to think about implementation is when designing the trial and that failure to think about implementation then can make it very hard later on.  In short, what are the design decisions we should be making to make implementation easier?
